You could have a bad stereo I/O card that could cause this. I had one fail way back in the day. Not common but it can happen. If you are looking to buy a used one, make sure it has the Apogee clock card in it. They are getting really hard to find after the fact and command a fair sum of money when you do find them. It is a necessity to have this card if you plan on hooking this up to any other digital gear.
Did I steer you over here from Gearslutz?

i think you did mate, many thanks. no this desk doesnt have an apogee card but ive read i should be able to get anyway with hooking it up to digital gear with the stock card although not very effective - ive seen some apogee cards on ebay so dont mind forking out.

Do you know what cards are in it now, F/X, I/O? I seem to remember clocking to the embedded clock if using the ADAT lightpipe hookups but I also seem to recall it being sort of flaky. I would recommend running with an Apogee clock card if I were in your shoes. It allows much more flexibility in your hook up choices.
