Fan & Heat Sink - How to Remove?

Hello,

My CPU Fan on top of the Heat Sink is very noisy. I can not figure out how to remove it. The Heat Sink has a lever on the side facing the front. When the lever is up the Heat Sink moves a bit but I cannot remove it.

Anyone know how remove the Heat Sink and Fan?

Can just the Fan be removed and replaced?
